Program Description
Project UNITE aims to address the needs of transition-age youth (TAY) by focusing on early intervention and provision of mental health supports for young adults with co-occurring developmental disabilities and mental health disorders and their family members.
Project’s Goals include:
- Implementation of mental health screenings
- Continuation of TAY Collaborative
- Development of wellness/drop-in center
- Development and implementation of Parent Mentor Program
Anticipated Outcomes include:
- Increase in the number of mental health screenings offered at WRC.
- Increase the number of young adults participating in psycho-educational interventions at a wellness/drop-in program in order to increase their adaptive behaviors, community integration, and decrease crisis situations.
- Enhance skills of parents and caregivers of young adults with dual diagnosis in system navigation, coping skills, sense of empowerment to effect change or obtain appropriate services for youth, and/or ability to address specific problems presented.
